County Form Information

The state requires that all septic tanks be pumped at least once every three years. Every three years, you will receive a notice in the mail from your local health department; this notice is referred to, by us, as a county form.


If it has only been one or two years since the septic system has been pumped, you can mail your paperwork to Josh S Bergendal Septic Service LLC at 7229 North County Road F, Edgerton, Wisconsin, 53534. We will fill it in with the most recent date and forward it to the county. You can include your check for $15 to the health department, and we will include it with your form, or you can mail your check to the health department separately.


If it has been three years since your septic tank has been pumped, you will need to call Josh and make arrangements for pumping. You can reach us at (608) 754-7816. After the work is done and the bill has been paid, we will fill out your paperwork and forward it to the county.

If you are not home when Josh comes to pump the septic system, you have options:

A. Leave a check for the pumping on the door with the county paperwork for us to leave filled out for you to mail in yourself

B. Mail your payment and your county paperwork to us and we will forward it to the health department.

At this time, there is no additional charge to fill out your county paperwork.

However, we will not fill out your county slip until your bill is paid in full.
